1. The purpose of the meeting:
The purpose of a 504 meeting is to review and discuss any accommodations or services that need to be provided in order to ensure that students with disabilities receive the appropriate educational support they are entitled to under the 504 Plan. This includes discussing any modifications, adaptations, and/or aides needed to provide equal access and educational benefits.
2. Attendees:
The attendees of the 504 meetings should include representatives from the school, parents/guardians of the student, and the student (when appropriate). Other participants may also be invited if needed or requested.
3. Review current documentation:
At the beginning of the meeting, any relevant documents and records should be reviewed to gain a better understanding of the student's needs and current level of functioning. This includes any assessments conducted by qualified professionals, medical/mental health records, reports from teachers, etc.
4. Review accommodations & services provided thus far:
The team should review all accommodations & services that have already been implemented to determine their effectiveness and to identify any areas where additional support may be needed.
5. Set goals:
The team should use the information gathered to set manageable and measurable goals for the student’s progress. This includes academic, social, and behavioral goals or objectives that are appropriate for the student's age and grade level.
6. Identify necessary accommodations & services needed to meet set goals:
Once the team has established their desired outcomes for the student, they can then discuss what type of modifications, adaptations, and/or aides need to be put in place to help ensure those goals are met.
7. Signatures:
Once all parties have agreed on the accommodations & services that need to be provided, signatures should be secured from the school representatives, parents/guardians of the student, and (when appropriate) the student.
8. Follow-up:
It is important to ensure that all accommodations & services agreed upon are being implemented in a timely manner. The team should designate someone to monitor progress and keep track of any changes made over time. It may also be necessary to schedule follow-up meetings on a regular basis in order to review the effectiveness of new or existing treatments and determine if any adjustments need to be made.
