1. Proof of Medical Insurance.
The patient and/or their family should bring proof of medical insurance to the nursing home. This can be in the form of a card or a letter from the insurance company.
2. Social Security Card.
The patient and/or their family should bring the patient's Social Security card to the nursing home. This will help ensure that the nursing home has the correct information on file for the patient.
3. Medicare or Medicaid Card.
If the patient is covered by Medicare and/or Medicaid, they should bring their card to show proof of coverage. This will help ensure that the nursing home has all the necessary information on file for billing purposes.
4. State Driver's License or Photo ID Card.
The patient and/or their family should bring a valid state driver's license or photo ID card to the nursing home so that the facility can verify their identity.
5. Copies of any Directives for your Physician.
If there are any advance directives from a physician in place, copies should be brought to the nursing home to make sure they are aware of them and understand what care is needed.
6. Power of Attorney Documentation.
If the patient has named someone as their power of attorney, that documentation should be brought to the nursing home so that they can grant them access to make decisions on the patient's behalf.
7. Living Will Documentation.
Any living will documents that are in place for the patient should be brought to the nursing home so that everyone is aware of their wishes regarding medical care and treatment.
8. Advance Directive Documentation.
In addition to a living will, any other advance directives or healthcare proxies should also be brought with you to the nursing home. This ensures all parties involved understand what type of care and treatment is desired by the patient.
9. Birth Certificate copy.
A copy of the patient's birth certificate should be brought to the nursing home for identification and verification purposes.
10. Marriage License (if applicable).
If the patient is married, a copy of their marriage license should be brought with them to the nursing home as well.
11. Medical Records from the Hospital.
Medical records from any recent hospital stay should also be provided to the nursing home for review prior to admission. This will help ensure all care received while in the hospital is documented on file at the facility.
