If submitting through email, make sure to scan the photo at a high-resolution for best results.
4. Proof of Identity:

7. Fee Payment Receipt:

8. Parental Consent Form:
If the applicant is below 18 years of age, a parental/guardian consent form must also be submitted with the passport renewal form.
