1. Driver Application:
This document is used to gather basic information about the driver, such as contact information, driving history, and experience.
2. Employment Agreement:
This document outlines the terms of employment between the driver and the employer. It should also include any specific duties that will be assigned to the driver.
3. Medical Certification/Examination Certificate:
Drivers must obtain a medical certificate from a medical professional before they can drive commercially. The certificate must be renewed every two years to remain valid.
4. Drug & Alcohol Testing Results:
Drivers must participate in periodic drug and alcohol tests to ensure they are not impaired while operating a vehicle.
5. Road Test Results:
After passing the medical and drug/alcohol tests, drivers must take a road test to demonstrate their driving proficiency.
6. Driver Training Program Certificate:
Drivers must complete a driver training program to be qualified for commercial driving. The certificate should show completion of the program and any additional training required by law.
7. Motor Vehicle Record (MVR):
An MVR is used to track a driver’s driving history, including tickets, accidents, violations, and suspensions or revocations of license.
8. Skill Performance Evaluation Certificate:
If the driver has any physical limitations that could affect his ability to drive safely, he may need to obtain an SPE certificate which assesses the driver's fitness for duty.
