This is the first page of the form and it contains general information about the form itself, such as the date it was filled out, who filled it out, and who it is for.
2. U.S. Refugee or Asylee Status
This section asks for information about the person's current immigration status. They must check one of the following boxes: refugee, asylee, parolee in removal proceedings, or other.
3. Identification Information
The individual must provide their full name, date of birth, place of birth, gender, and current mailing address in this section. They will also need to provide information about any other names they have used, as well as their Alien Registration Number (A-Number) if they have one.
4. Eligibility for Public Assistance and Benefits
This section is used to determine if the individual is eligible for certain public assistance programs or benefits. The form asks questions about the person's employment history, criminal history, and whether or not they have been a recipient of public assistance in the past.
5. Declaration of Immigration Status
In this section, the individual must declare their current immigration status. They must check one of the following boxes: lawful permanent resident, conditional permanent resident, refugee, asylee, or other.
6. Signature and Date
The individual must sign and date the form in this section.
7. Certification
This section is for the individual's certification that all of the information provided on the form is true and accurate to the best of their knowledge.
8. Documentation of Employment Authorization
If the individual is seeking employment authorization, they must provide documentation in this section. This could include a copy of their refugee travel document or an asylee asylum card.
9. Declaration of Self-Sufficiency
In this section, the individual must declare that they are self-sufficient and will not become a public charge. They must check one of the following boxes to indicate how they will support themselves: employment, sponsorship, and other means of financial support.
10. Additional Documentation (If Needed)
If any of the information provided on the form needs to be clarified or if there is additional information that is needed, this can be done in this section.
11. Family Information
This section asks for information about the individual's family members, such as their name, relationship to the individual, date of birth, and place of birth.
12. Financial Information
In this section, the individual must provide information about their finances, such as their income, assets, and debts.
13. Residence Information
This section asks for information about the individual's current place of residence, such as their address, how long they have lived there, and who they live with.
14. Education and Training Information
The form asks about the individual's education and training in this section, such as their highest level of education attained and any job-related skills or training they have received.
15. Work History Information
This section asks for information about the individual's work history, such as their current occupation, employment history, and skills related to their occupation.
16. Immigration Violations (If Any)
If the individual has any immigration violations on their record, they must list them in this section.
