A valid Social Security Card or government-issued document proving the individual’s social security number must be presented.
3. Two Documents Proving Residency in Illinois:
These can include a recent utility bill, credit card statement, lease agreement, etc., which shows the applicant's current address in Illinois.
4. Proof of Name Change if Applicable:
If an individual has changed their name since the last time they applied for a driver’s license or ID card, proof of this change must be provided (such as a marriage certificate).
5. Fees and Payment Information:
A fee must be paid at the time of application to cover the cost of processing and issuing the REAL ID.
6. Identification Number (if applicable):
If an individual holds a driver’s license or state identification card, they must provide their number.
7. Digital Photograph:
A digital photograph will be taken at the time of application for use on the REAL ID card.
8. Form DS-11 (if applicable):
If applying for a first-time driver’s license, form DS-11 must also be filled out and provided along with other documents.
